Daniel's therapy evaluation results
Date of eval - 3/12/08
Chronological age - 30 months
Cognitive ability - 27 month, with heavy scatter to 36 months
Communication -
Expressive - 24-27 months
Receptive - 33-36 months
Articulation - atypical (this is where he still needs work)
Social/Emotional - 28 mths with scatter to 36 months
Adaptive - 30 months
Gross Motoe - 30 months
Fine motor - 27 months with higher scatter.
Daniel was initally evaluated on 9/20/07 and determined to have a language delay characterized by a limited sound repertoire. Since initiating speech therapy in october, Daniel has improved from the 7-8 month level (scattered to 16 months) to the 24-27 month level in March. He has demonstrated steady progress over the course of the past 6 months and rapid progess in the past few weeks. He now imitates a variety of single words and 2-word phrases with high frequency. His vocabulary has grown beyond the average 50 word level expected at 24 months of age.
Daniel is also using a variety of verbs including "ing" verbs texpected around 36 months of age. He is using language to get his wants and needs met and maintaining his language skills across settings. He no longer demonstrates a delay in his language skills.
Although Daniel's language skills are now age-appropriate, the clarity of his speech is affected by numeroug articulation errors. Daniel inconsistently presents with vowel distortions, especially while producing dipthongs /au/ as in "house", /oi/ as in "toy", and /ai/ as in "ice". Additionaly he demonstrates errors during production of 2-3 syllable words like glasses, balloon, water, and hot dog. His error patterns during production of these words include: weak syllable deletion (bun/balloon), assimilation (dog-dog/hot dog, gagges/glasses), and consonant substitution (waja, water).
SOme additional sounds Daniel inconsistently produces include: w, h, n, and K, as well as voicing errors such as g/k and b/p (gock/clock, baber/paper).
She recommends his 2x/month group therapy and also 2x/month home speech sessions for the next 6 months. He graduates out of the program and into the school system when he turns 3 in October. She is hopeful that he will not need therapy after that.
